The care management program helps older adults live in their own homes with confidence and as much independence as possible.
 
 
The care management program helps older adults live in their own homes with confidence and as much independence as possible. Professional care managers assist clients with navigating the challenges of aging by providing connection to supports and resources. The goals of the program are:
  • to help participants successfully meet their health and psychosocial needs in order to maximize their ability to live in their own homes with as much independence as possible for as long as possible.
  • to reduce admissions to more acute levels of care including hospitals, assisted living facilities, and skilled nursing facilities.
Professional care managers provide strengths-based, person-centered care to program participants. This care involves comprehensive assessment, the identification of client-centered goals, strategic problem solving and connecting clients with needed supports and community resources. For eligible Holocaust survivors, this includes assistance with application to The Blue Card, which provides holiday and birthday stipends and funding for expenses such as homecare, dental care, hearing aids, and medical transportation.  JFS care management services include home safety assessment, linkage to home care resources, monitoring of health and well-being, health literacy, assisting with transitions between levels of care (e.g. return home after a hospitalization), or end of life planning. Direct financial and other assistance with basic needs is also provided (e.g. food and utility assistance, assistance with home safety modifications such as grab bars, medical transportation, etc.).
